The Astrea is dispatched to Apra, an inhabited planet near the Federation and Romulan border. The planet is home to Humans, Romulans, and Esorians, and is home to one of the larger Romulan colonies that was relocated to Federation territory. The Romulans have mostly kept to themselves since being relocated to the planet, though there is some interaction between their colony and the others.

A viral fever has broken out among the Romulan population. It does not appear to be affecting the Humans or Aprarians, despite the fact that Aprarians have a similar enough physiology that other pathogens are easily communicable. The Romulans are suspicious that this is a biological attack meant to weaken or exterminate their population. The Esorians are suspicious that Romulans have brought this disease to their world and that the Romulans are a threat to their own livelihood. As one of the smaller Federation worlds, the Esorians often feel their needs and concerns are overlooked by the Federation and Starfleet.
